Surgical Treatment for Infective Endocarditis in Infant: A Case Report

Abstract

Introduction : Infective endocarditis is still a problem in Thailand.  Majority of these cases in children were managed by medical therapy.  So, surgical treatment for infancy child of infective endocarditic is very rare in our unit.

          Method : A case of infant with infective endocarditis which performed surgical treatment was reviewed for clinical features, management and outcome.

          Result : Seven month old female infant, 4.8 kilogram of weight admitted in university hospital with fever and marusmus condition.  Hemoculture found Staphylococcus organism and immunoglobulin test was positive for Cytomegalovirus.  Echocardiogram recorded perimembranous VSD with oscillating mass confined from tricuspid valve, right ventricular chamber through pulmonary valve.  Surgical treatment was performed under CPB to removal of vegetable mass, Tricuspid valve repair and closure VSD with pericardial patch.  The patient stayed 26 days after operation before went back home.

          Conclusion : In compromised infant, infective endocarditis can be occurred.  Right side heart failure from vegetable mass should be managed by surgical removal.  Team approach is mandatory for success management.
