Assessment of BEAT Simulator Training System for Coronary Artery Anastomosis; the Interim Result

Abstract

Purpose: In the field of cardiovascular surgical training, simulation-based approach as the Off-The Job Training (Off-JT) has been gathering growing attention in the aspect of modalities as well as evaluation of the outcomes. We have developed a simulator system for coronary artery anastomosis (BEAT System) using YOUCAN (graft model) and have conducted 13 series of large scale contest-style sessions and 3 Boot Camps(BC) for last 3 years. In this report, the evaluation this BEAT system from our consecutive 16 series of training sessions.

Method: We have organized two series of BC, a large scale session at the time of related congress (A) and small group teaching 4 hours-session with 6-10 trainees and 4-6 senor instructors. The total number of attended trainees were 169 in A and 23 in B-Session. The A Sessions were conducted as a contest, and B as for small group teaching with instructors. The quality of anastomotic skill has been evaluated for each trainee by attending (or by video) senior instructors using score system and also by fluid computational assessment (FCA) for each anastomosis for feed-back.

Results: In the A-Session, total 39 trainees were selected as award winner in 13 series. In B-Session, the summery of quality assessment by Video record showed score 3 independent operator level (excellent) in 17%, score 2 (fair) in 87%, and score 1 (poor) 0%, respectively (N=12). In B-Session, the trainee’s response was well appreciated by direct on-site teaching as well as later report of CFA.

Conclusion. The BEAT System has been well accepted as the main modality for coronary artery bypass simulator training in Off-JT particularly under senior instructor. The direct skill-evaluating score as well as CFA evaluation may be further refined for standardization and quality improvement.
