Functional State of Cardiovascular System in Different Periods after Ischaemic Stroke

Abstract

The relevance of the research. Vegetative-visceral dysfunction often occurs in acute cerebrovascular accidents that violate the regulation of the cardiovascular system, which greatly exacerbates the neurological condition of patients after ischemic stroke (IS).

Purpose of the study.To explore the features of hemodynamic and autonomic regulation of heart rate in different periods after IS, as well as to clarify their connection with the functionality of the heart.

Data and methods for study.The study were carried out on male rats of Wistar line.Individual functionality of myocardium was determined by using stress echocardiography with dobutamine. Focal IS was created by local photochemical vascular thrombosis of the cerebral cortex over the parietal region of the left hemisphere of the brain, which was then confirmed by MRI. The analysisof the heart rate variability (HRV) was conducted in different periods of the IS. Echocardiography was performed with help of Mindray M5 unit, sensor of 10 MHz (firm Mindray, China).

Results and their discussion.Stress test with dobutamine made possible to identify animals with high (I group) and low (II group) functionality.Analysis of the resultsobtained in the acute phase of stroke and hemodynamic study of HRV showed no differences in related indicators in the control animals groups I and II. In the subacute stage of IS nature of the hemodynamic changes in group I and group II was different. Thus, in animal group I pulmonary vascular resistance (PVR) decreased, but cardiac output was higher than the control level.Increase in total peripheral vascular resistance and PVR in animals with low functional capacity of heart occurred against the backdrop of diastolic heart function. There were no changes in hemodynamics in animals group I after 35 days after IS (recovery period), whereas in group II changes were saved, indicating the prolonged violation of diastolic heart function.
