First Result of Minimally Invasive Mitral Valve Surgery at Viet-Duc Hospital

Abstract

Objective: Minimally invasive cardiac surgery (MICS) is a revolution of open heart surgery in the world since more than 10 years. Viet Duc university hospital is a biggest surgical training center for the Nord of Vietnam. MICS was applied at Viet Duc hospital since April 2014. The report to introduce initial results and some experiences of MICS for mitral valve surgery. Methods: prospective study of all minimally invasive mitral valve surgery at Viet Duc hospital from April 2016 to September 2017, about parameters of preoperative pathological characteristics, surgical techniques and early postoperative results. Results: including 80 patients, 62,5% was female; average age was 41,5 ± 12,8 years old (35-65). General surgical procedure included 35 cases of mitral repairing and 45 cases of mitral replacement, with/without tricuspid valve plasty. MICS’s techniques: arterial and venous cannula through femoral artery and vena; 4-6cm of small right thoracotomy; MICS system installation with thoracoscopic camera support; clamp aorta with Chitwood clamp, cardioplegia with custodiol-HTK and doing all of surgical movement for mitral valve with MICS instruments. Technical success was 100%. Cardio-pulmonary bypass time was 158,5±35,5 minutes; aortic clamp time was 115,4±52,2 minutes. Post operation: no mortality; 85% needed artificial ventilation under 3 days with average ventilation time was 1,5±2,4 days. Hospitalization time was 12,6±6,2 days.Complications: 3 cases of bleeding (3,75%); cerebral accidence in 1 case (1,25%); heart failure in 3 cases (3,75%). Postoperative ultrasound examination saw good results. Programed control, from 1 month to 3 years, has a stable result like classical surgery. Conclusions: MICS for mitral valve installed successfully at Viet Duc hospital and this procedure can apply widely for other type of surgery and to other heart surgical center in Vietnam
