Coronary-Cameral Fistula Closure Using Different Types of Occluding Devices

Abstract

Objective. To determine results of endovascular occluder closure of the coronary-cameral fistula.

Materialsandmethods.In SCCVS named after A.N. Bakulev we have an experience of endovascular closure of the coronary-cameral fistula in 41 patients in which in 15 cases closure was performed with occluders. Age of patients was from 10 months to 57 years (16,3+5,8 years). In 11 patients coronary-cameral fistula opened into right cameras of heart (right ventricle, right atrium or pulmonary artery), in 3 patients- into left heart. Implantation of the occlude was performed through venous access in 10 patients and atrial access in 5 patients.

For coronary-cameral fistula closure in 15 patients were implanted 16 occluders AMPLATZER: ADO I -12; ADO II- 1; ADO II additional size -1; Muscular Septal Ventricular Occluder-1 and Plug Occluder - 1.

Results.After procedure, complete fistula closure was archived in 13 patients, and in 2 patients- insufficient residual flow was registered. In long term postoperative period (from 1 to 6 years) all patients were asymptomatic. In one case- asymptomatic thrombosis of circumflex artery was registered. In 2 cases in long term postoperative period endovascular interventions were performed after coronary-cameral fistula closure: in 1 case- stenting of the right pulmonary artery at the the fistula drainage site, in 2 case- closure of the closure of ventricle septal defect.

Conclusion. Transcatheter closure of large coronary-cameral fistula using different types of occluders may be considered a safe and effective method of treatment.
