Surgical Treatment of Patients with Combined Atherosclerosis of Coronary and Carotid Arteries

Abstract

STUDY OBJECTIVE: To present the results of surgical treatment of patients with multifocal atherosclerosis based on a differential approach of surgical strategy.

MATERIALS AND METHODS: During the period from 01/01/2015 - 01/01/2017 in FCCVS n.a. S.G. Suhanov, Perm we operate 232 patients with combined atherosclerosis of coronary and carotid arteries. In 83 patients (36%) we performed combined operation CABG with CEA and in 149 (64%) patients we performed underwent stage treatment. The average age of patients is 63 ± 12.1 years. Postinfarction cardiosclerosis was in 104 patients (45%), previous ischemic stroke in 75 patients (32%). Arterial hypertension (98%). Diabetes mellitus was diagnosed in 33 (14%), obesity in 41 (18%) patients. Depending on our differential approach we select 83 patients (36%) on simultaneously and 149 (64%) patients on stage treatment.

RESULTS: In a group with combined surgical correction we do not have cases of perioperative stroke, myocardial infarction. In the group of stage treatment there was 1 case of MI after CEA (0.6%) and 1 case of stroke after CABG (0.6%). Mortality in all group was 0.

CONCLUSIONS: Combined surgical treatment of coronary and carotid arteries atherosclerosis does not increase the risk of postoperative complications in comparison with stage treatment. The proposed approach of treatment methods for combined lesions of the carotid and coronary arteries based on the differential approach is safe and allows to adequately eliminating the lesions.
